The Eileen Head is a demonic manifestation meant to reciprocate or mirror Henry's voyeuristic tendencies towards Eileen, whereby Henry used to peep and spy on Eileen in her room, so now, a monstrous, decapitated Eileen Head uses its giant eyeballs to follow and track Henry around the room to show him exactly what it feels like to be peeped upon.
The Eileen Head (unofficial name) is a non-combative "monster" of sorts found in a room within the Otherworld version of St. Jerome's Hospital in Silent Hill 4: The Room. The Eileen Head is simply Eileen Galvin's head, but her skin is scarred and diseased-looking and her eyes are bloodshot.
