Joy mask is required to get both alternate endings, and I hear Terry's mask is needed to eventually get the "No Friends" achievement but I can't confirm that. Take your mask off, and literally every NPC in the game will attack you.

No I'd be happy to share! I followed this mask tutorial on Deviant Art. It uses a paper mache base and the top part is made with paper clay. Paper clay is sand-able after it dries, which is how you get it so smooth.
