Guts has been using the Dragon Slayer to kill apostles for such an extended time that their blood and malice have tempered the sword, imbuing it with astral properties suitable for inflicting damage on astral beings.
Guts slays his first apostle with the Dragon Slayer. After roughly a month of recuperating and training, Guts sets to combating demonkind, brandishing a new set of armor along with a cannon-equipped artificial arm in place of his severed left arm.
The only reason Guts isn't his plaything like everyone else is because Guts has defied that control on an equally spiritual level, and even matched it by being Griffith's equal. That's always been my interpretation, at least.
After Guts dons the Berserker Armor, Grunbeld was taken off guard by Guts' new found strength and assumes his apostle form. Grunbeld attempts to keep Guts and his party from escaping, but he is blocked from pursuing them further by the wall of fire Flora conjured in her final moments.

This would never ever happen (it can't happen, for that matter), but theoretically let's say that Guts somehow decides to sacrifice someone and become an apostle. This happens at where we are in the story now. What would he look like, and what would he be capable of?
But I’ve often found myself wondering what could possibly push Guts to the point where he would use the behelit and make the sacrifice required to become an Apostle. Guts hates Apostles and much of the story revolves around him hunting them down.
