Sacrifice Rooms are special rooms that contain a set of spikes in their center. Isaac can damage himself on the spikes for a chance to spawn a Chest or a Locked Chest. The chance is determined by the seed, meaning Isaac must damage himself on the spikes a set number of times for a Chest to spawn.
The Sacrifice Room is a special room added in the Wrath of the Lamb expansion. It is a room with the same backdrop as The Depths, but with numerous pools of blood scattered around. In the center of the room, there is a Spike tile.

Sacrifice Rooms have a higher chance to spawn if Isaac has more health to sacrifice (if Isaac's total health (red, black, and soul hearts) is equal to or greater than his number of heart containers). Sacrifice Rooms now can pay with a much wider variety of rewards.
A helpful strategy for the angel bosses is trying to lead them into the spikes in the middle of the room which will damage them. If my run is going well, I like to give a couple hits to the sacrifice room to try and get the angel room item or 7 soul hearts as a bulkwark for the next floor.
